JAKE REEVERTS, PT, DPT, LAT, ATC, CSCS

Dr. Jake grew up in Princeton, IL, and attended Olivet Nazarene University in Bourbonnais, IL, where he received his Bachelors in Athletic Training with a minor in Exercise Science. During his time at Olivet, he worked countless hours rehabbing collegiate athletes of all sports from day one injury to returning to the field. Following his time at ONU, Jake received his Doctorate in Physical Therapy from the University of Evansville, learning from world-class clinicians. After graduation, Jake moved to Vincennes, IN, where he worked for Good Samaritan’s Sports Performance Department and served as Vincennes Lincoln High School’s athletic trainer, covering many sporting events and treating injuries on the field and through their rehab in the clinic. He has since returned to the Evansville area and is passionate about treating the athlete and active population.
Jake decided to leave the corporate physical therapy world to open his own cash-based clinic because he wanted the freedom to provide more personalized care to his patients. In the corporate setting, he often found himself limited by insurance constraints and strict productivity targets, which made it difficult to truly focus on what each individual needed. By starting his own clinic, Jake is able to offer one-on-one, tailored treatments in an environment where patients come first—without the restrictions of third-party payers. This allows him to build stronger relationships with his patients and help them achieve their goals more effectively.

Jake and his wife, Miranda, met in the Doctor of Physical Therapy program at the University of Evansville. Miranda works as a pediatric physical therapist with Indiana First Steps. The two reside in Newburgh, IN, with their son, Parker. When not at work, you can expect to find Jake traveling with his family, competing in disc golf tournaments, attempting DIY projects around the house, or working out at the gym.

Jake is always working on his disc golf game whether that be improving his form or distance. He is always up for playing with a new or experienced disc golfer!
