During your initial visit, you can expect a thorough examination lasting about 90 minutes. This will involve reviewing your medical history and symptoms, discussing your goals, conducting a detailed musculoskeletal and movement assessment, and explaining the results along with a proposed treatment plan. Treatment will begin in the first session, which may include hands-on manual therapy, corrective exercises, or other interventions based on the evaluation. After the session, you’ll receive a home program designed to reinforce the progress made and support your goals.

No, you don't need a referral! In Indiana, under the Physical Therapy Practice Act, you have direct access to physical therapy services without a referral. Call us first!

We provide in-home evaluation and treatment sessions within a 20-mile radius of our clinic, located at 4944 Old Indiana 261. This service area is included in our standard pricing. For appointments outside this 20-mile radius, a travel fee of $0.70 per mile will be applied for both the trip to your home and the return trip to our clinic.

To offer personalized, one-on-one physical therapy without insurance limitations, Dr. Jake is an out-of-network provider. This approach allows for faster access to services, no waiting for insurance approvals, more flexibility in treatment sessions, and in many cases, lower overall costs for your care.
However, with many non-Medicare plans, you can file a self-claim, but it is best to confirm this with your insurance company for a definite answer. Reeverts Rehab will provide you with a superbill that you can submit to your insurance, which may apply to your out-of-network deductible and could be eligible for reimbursement, dependent on your plan.

Absolutely! At Reeverts Rehab, we offer a variety of treatment package plans to suit your needs. These options can be discussed in detail with Jake during your initial evaluation. Choosing a package plan is often recommended, as it supports consistency in reaching your fitness goals while also offering long-term savings on per-visit costs.

Cash, card, check, or HSA/FSA are all accepted payment methods. Payment is collected at each treatment session, unless a package is purchased ahead of time.
